At this time, Elevate Life Solutions PLLC is private pay for couples therapy. We accept Blue Cross Blue Shield Insurance for individual therapy.
What are your rates?
15-minute complimentary consultation
75-minute initial session at $190.00
60-minute individual session at $150.00
60-minute couple session at $175.00
90-minute extended sessions at $225.00
What is the No Surprise Act?

What does this mean for me?
If choosing to waive insurance or opt for “private pay”, then it is your right to a Good Faith Estimate within 3 business days of your scheduled visit.
What is included in a Good Faith Estimate?
A good faith estimate includes expected charges for the health care services. This form will be included in the intake packet that is sent via email. Instructions will be given to complete the intake packet before the first appointment to assist the clinician in preparing for the initial session. Are you virtual only?
Elevate Life Solutions PLLC offers in-person sessions along with virtual sessions. Combinations can be accommodated upon request.
How do I get started?
Email Elevate Life Solutions PLLC at info@elevatelifesolutions.net and you will get a response within 1 business day. The email is intended to obtain your contact information, a brief understanding of your reason for treatment and set up either a free consultation or a first appointment. During the process, an intake packet will be sent via email to complete BEFORE the first appointment including a brief intake form, credit card form, informed consent form, demographic information, and Good Faith Estimate, if applicable.

The initial session’s goal is to welcome you to Elevate Life Solutions PLLC. Elevate Life Solutions PLLC wants you to feel safe and comfortable as the healing journey begins. A review of all the policies and procedures will be conducted to ensure a thorough understanding before consenting to treatment. Next, Elevate Life Solutions PLLC, will begin the process of gathering information to create a unique treatment plan for the journey.
